Either Schedule D or Form 4797, including Form 4562, flowing to Page 1 of Form 1040.

If it was personal property, you would just have to fill out the Schedule D with your 1040 form. If it was business property, you would have to fill out Form 4797, Schedule D, and Form 1040.
